The AusACPDM is pleased to offer a series of awards to honour our outstanding members and contributors to society.

Nominations for the AusACPDM Awards are now closed.

Award Categories	The Award is for:
Advocacy Achievement Award	Making a significant contribution to advocating for people with lived experience of disability
Mentorship Award	A person who mentors others to improve services and care, promote professionalism and support ongoing education and research
Person with a Lived Experience Award – Outstanding Contribution	A person with cerebral palsy or developmental disability who promotes the health and wellbeing of others
Local Hero Award	A person with cerebral palsy or developmental disability, aged from 12 years, who resides in the Cairns area
Transformative Practice Award	A clinician of any discipline who encourages evidence-based practice and translates research into practice

How to nominate someone?

Each conference year, a call will be made for nomimations, to do this, we will ask that a letter (Max 400 words) is submitted outlining:

Which award it is that you are nominating for
Name of nominee
Nominee’s email and contact number
How long, and in what capacity have you known the nominee
Nominee’s contribution
Your name, email and contact number
